Cancer is the second leading cause of death in the state of Maryland. The American Cancer Society estimates that there were over 10,320 cancer deaths in Maryland during 2009. The next installment of the Maryland Comprehensive Cancer Control Plan is being developed. Because of the current progress with Maryland’s cancer control efforts, graphic design, layout and editing services are required to complete the next version of Maryland’s Comprehensive Cancer Control Plan, its Executive Summary and a Pull-out guide.

1.1.2 The Maryland Department of Health and Mental Hygiene (DHMH), Family Health Administration (FHA), Center for Cancer Surveillance and Control, Office of Comprehensive Cancer Control (CCSC) is issuing this Request for Proposals to procure the services of a graphic artist and/or design firm to provide complete graphic design elements for the Maryland Comprehensive Cancer Control Plan, its Executive Summary and a Pull-out guide.
1.1.3 The Cancer Plan document will be formatted as a hinged ring binder. The binder will include dividers with tabs for each chapter and all indices. The Executive Summary may be glue bound or inside center staple format (see current MCCCP Executive Summary as an example). The pull out guide format will be a single sheet that unfolds and is attachable to the larger binder with temporary and easily removable glue that will not damage the sheet or the binder components. This sheet may be three-hole punched, therefore formatting must account for this.
A pre-proposal conference (Conference) will NOT be held. Prototypes of products similar to the final products will be available for Offerors to view, by appointment only, at 201 W. Preston St, Baltimore, MD 21201, room 400, by calling the Contract Monitor, Felicia Plummer at 410.767.6213.
All questions that are not previously answered in this RFP are to be submitted in writing to the Procurement Department to the attention of Sharon Gambrill.
